Insurance information

Unfortunately, insurance carriers in the state of Illinois do not cover ketamine infusions, as this leading-edge treatment is currently considered to be an off-label use of the drug. If you decide to pursue treatment, infusions will be self-pay and due at time of appointment. Upon request, however, we are able to provide you with a Superbill, a detailed report of your visit, that you may submit to your insurance carrier for direct reimbursement. Reimbursement is dependent on individual insurance carriers and individual plans and Midwest Alternative Treatment Clinic cannot guarantee reimbursement of any kind.

Self-Guided Ketamine
Infusions for
Mental Health

$350 per 45-60 minute infusion

  • Empowering individuals with the agency for changing their lives when other treatments have failed.
  • Provides a rapid onset of action and upwards of 80% response rate and a 20% remission rate for individuals suffering from depression, anxiety, obsessive-compulsive disorder, post traumatic stress disorder and addiction.
  • Patients are monitored on a cardiac monitor as well as pulse oximetry under close supervision by highly trained medical personnel for entirety of visit.
  • Medications for nausea, blood pressure, and anxiety control are provided as needed at no additional cost adding to our goal of affordable ketamine treatment.
  • Please allot for an additional 30 minutes following your infusion for monitoring and reintegration.

Ketamine Infusions for
Chronic Pain

2 Hour Pain Infusions: $600

3 Hour Pain Infusions: $800

4 Hour Pain Infusion: $1000

  • Up to 4-hour long ketamine infusions depending on individual needs and tolerability.
  • Patients are monitored on a cardiac monitor as well as pulse oximetry under close supervision by highly trained medical personnel for entirety of visit.
  • Ketamine is generally considered to be a non-addictive pain management option and have a lower risk of dependence compared to other substances, such as opioids or stimulants.
  • Please allot for an additional 30-60 minutes following your infusion for monitoring and reintegration.

Ketamine Assisted Psychotherapy
(KAP)

  • 1-2 Onboarding Sessions with Therapist Prior to infusions – $150 each self pay or billed to insurance
  • KAP IV Ketamine Sessions (2hr) – $500 per session. Includes pre-infusion session with therapist, ketamine infusion, and post-infusion integration session.
  • KAP Oral Ketamine Sessions (2hr) - $400 per session. Includes guided sessions with trained KAP therapists.

*A minimum of 3 sessions is required, however the total number of sessions is determined by the patient and care team. Our standard treatment plan is six sessions.

Oral Ketamine for Home Use

  • $225 Initial Telehealth Visit
  • $175 for subsequent refills (monthly visit required for refills)
